NOTE: This form must be filled out completely. Application for a permit to operate does not guarantee that a permit will be granted. Permit approval is based upon compliance with state and local health ordinances. Contact the Health

All permits expire Jan. 31 each year. You will be mailed an annual renewal form each year to be completed and returned with the permit fee. Permits will not be issued and late fees may be charged if an incomplete application is returned and received after due date. All permits are non-transferable upon sale, transfer of ownership/location and permit fee will not be refunded.

Make checks payable to City of San Angelo.

Food establishment permits are $150 per year. Each additional service area is $25, prorated to Jan. 31. Call for prorated fee if yours is a new establishment.

Nonprofit agencies are exempt from annual fee only, not from late fees.

Late fees of $25 are applied the day after the expiration date and for each month thereafter. Fees can be mailed to:
